Insurance and Billing

At the time of your visit, patients need to provide our office with the correct insurance information, including your insurance card to be copied. Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services. We accept Visa, Mastercard, Discover and Care Credit. Care Credit is a convenient, monthly payment program for yourself and your family specifically designed to pay for healthcare expenses not covered by insurance including co-payments, deductibles and elective treatment and care. For more information about Care Credit payment plans, please contact our center.

For your convenience, we accept Medicare and most insurance plans. In either case, you'll get an easy-to-understand bill from us, and you'll also get separate bills from your surgeon and anesthesia provider. We welcome self-pay patients, and we'll be happy to discuss payment arrangements.

Please don't hesitate us to call us with any insurance or billing questions: (320) 258-6620. Our staff is eager to address any questions or concerns you may have.

Our in-network insurance plans include:

  • BCBS - All Products
  • First Health
  • Health Partners
  • Medica
  • Medicaid
  • Medicare
  • MMSI - Centra Care Health
  • Multiplan
  • PHCS
  • Preferred One
  • Select Care
  • U-Care

The Cost of Your Procedure

Because you are responsible for costs for covered healthcare services until you reach your deductible, you should comparison shop if you elect to have a nonemergency procedure.

You can use tools like Healthcare Bluebook and the Medicare Procedure Price Lookup (for Medicare patients).  Many health insurance websites also provide information on where to find in-network services.
